Victorian Integrated Care Online Training Resources

In order to support the implementation of the Victorian Integrated Care Model (VICM), there are 6 online training resources that provide readings, case studies, activities and videos that will extend their understanding of the key principles, concepts and approaches of patient-centred integrated care.

Who are the resources for?

All health, community and social services workforce are welcome and encouraged to use the resources. They can be completed at the user’s pace and in desired order.

The online resources extend the support available for those with a vested interest in implementing and supporting high quality integrated care in their local network. The resources are designed to be completed at a self-determined pace allowing you time to learn gradually, reflect on your current health services and care provision, plan for any improvements and incrementally implement your learning as you progress through the resources.

The resources can be accessed by visiting: www.victorianintegratedcare.training Users will be able to self-register and obtain their personal login details by clicking on ‘Create account’. This will take you to the ‘Create account’ page where you will enter your details, some of which are mandatory.

A certificate is available at completion of the resources.

A certificate is available at completion of the modules. The modules have been developed by AGPAL in collaboration with the Victorian DHHS and partners. They are consistent with training available to General Practitioners on the Health Care Home and NSW healthcare providers on the patient centred medical home.

Need help with the resources?

In addition to the online resources there are nominated Facilitators from the Victorian Department of Health and Human Services, Australian Disease Management Association (ADMA) and public health services (change managers) available to support your learning as you progress.

Victorian Integrated Care Forum

The Victorian Department of Health and Human Services held a one-day Integrated Care Forum in June 2018 at the Victoria University City Convention Centre. The event brought together over 140 people from across Victoria, with delegates from other states also attending. The forum featured a diverse line up of speakers, including international keynote speaker, Robin Miller, from the United Kingdom.
