OngoingBrief Summary:
Access Health and Community is a Community Health Service in the inner and outer east of Melbourne. Access to Community (A2C) uses a Volunteer Community Connector model, whereby volunteers are paired with people to build social confidence and capacity and to navigate community activities together.
Supports are provided by phone and in community. Our volunteers speak many languages and we aim to support community in culturally relevant ways. Our volunteers are trained and supported by a program specific Coordinator of Volunteers.
Referrals are received from GPs, wellbeing professionals, diabetes educators and clinicians. Self-referrals also welcome.
Who is this for?
People with connection to Boroondara, Yarra and Manningham who need support to become more socially connected. Aimed at people under 65 with potential to support CHSP eligible people over 65.
